Once the application form has been completely filled take a print out of the form and affix your signature.(should be done only by the account holder)
Kindly note Address change will be effected only if the first holder/first applicant remains same across all the products and services.

Send the request (Application form and self-attested document) through post to your nearest branch Click here. For Demat any one of the holder of the demat account or customer representative have to visit the DP servicing branch providing demat account modification services to submit the Change of Address form.
General Instructions for Demat account:
- The form is applicable only for NSDL Individual demat accounts. Trust, Partnership, Minor, Corporate and Custody demat accounts are excluded.
- The form is to be submitted to DP servicing branch only, providing account modification services.
- Any one of the demat account holder or customer representative has to visit the DP servicing branch providing account modification services to submit the Change of address instruction.
- The instruction has to be signed by all the holders of demat account as per the signature(s) of the holders updated in demat account.
- Minimum one foreign address is mandatory for NRI Demat account.
- Landmark should be prefix by words such as next to, opposite to, adjacent to, near to, behind, beside etc.
- Self attested valid proof of new address is required of first holder of demat account. In case if the instruction is for both the addresses ie. Correspondence & Permanent, then proof of address is required of both the addresses.
- The check box pertaining to “SMS Alert facility” to be selected / ticked.
- The original documents along with the self-attested photocopies are to be submitted to the DP serving branch for verification.
- The address mentioned on the form should match with the address mentioned on the proof of address. In case of mismatch in the address, additional proof of address is required.
- Correction / Alteration on the form should be authenticated by all the holders of demat account.
- Email Id. mentioned on the form will be updated only in the Client Master.
- All communication will be sent to customer Mailing/Correspondence or Permanent address as per the consent given by customer in the instruction.
(*) Incase trading a/c is not mentioned, then Demat account will be considered for identification of trading account and the change of address would be replicated in HSL Trading a/c.
